2. INTRODUCTION

The IAEA has gained more experience and success than any other international organizations in establishing tissue banks in developing countries and applying ionizing radiation for sterilizing tissue grafts, used in transplant surgery (in orthopaedic reconstruction, treatment for cancer, trauma and high velocity impact damage), the treatment of burns, leprosy and intractable skin wounds and pressure sore ulcers.

Thirty countries have been involved in this initiative: These are the following: In Asia/Pacific Region 14 countries: Australia, Bangladesh, China, India, Indonesia, Japan, Republic of Korea, Malaysia, Pakistan, Philippines, Singapore, Sri Lanka, Thailand and Viet Nam. In Latin America Region 7 countries: Argentina, Brazil, Chile, Cuba, Mexico, Peru and Uruguay. In Africa and Middle East 5 countries: Algeria, Jordan, Libya, Zambia and Iraq, and in Europe region 4 countries: Greece, Poland, Turkey and Slovakia.

Most of these countries are using radiation sterilization as a method of choice. Experts from Australia, Europe, Japan and USA have assisted the IAEA Radiation and Tissue Banking Programme.

In the countries involved with the IAEA Radiation and Tissue Banking Programme, as experience has been gained, the growth had been exponential. To the year 2001, more than 222,580 tissue allografts have been produced and used clinically. Taking the mean value of the cost level by Tissue Banks in USA, Europe as a re-charge for the processing, the value of these grafts can be placed at US $51 768 553. The total expenditure associated with the IAEA Radiation and Tissue Banking Programme over the same period was US $6 313 335 which includes a training cost of US $2 036 034.

One of the major advantages of the IAEA Radiation and Tissue Banking Programme is the exposure of surgeons in these countries to the newer methods of using allografts and creating a cultural change in their approach to surgical treatment in their country.

In Western countries the use of such grafts is now routine. The perceived need in a developing country is not always apparent, but as the technology is introduced, the health benefits become clear. As this occurs, the graft supply is far from being sufficient. The IAEA Radiation and Tissue Banking Programme has inspired this revolution in many countries and the growth henceforth will be exponential. Consequently, the future planning is designed to address this situation.

During 2001-2002 a group of documents has been prepared by many experts in different fields. These are:

  1. The International Standards on Tissue Banks
  2. The Code of Practice for the Radiation Sterilization of Tissue Allografts.
  3. Public Awareness Strategies for Tissue Banks

One of the major objectives of the IAEA Programme during 2003-2004 is to promote the use of these documents in the participant countries.

3. OBJECTIVES TO BE ACHIEVED

  1. To encourage the use of the following documents prepared through the IAEA Radiation and Tissue Banking Programme in the participating countries:

    a. The International Standards on Tissue Banks.
    b. The Code of Practice for Radiation Sterilization of Tissue Allografts.
    c. The Public Awareness Strategies for Tissue Banks.

  2. To train Tissue Bank Operators Managers and Doctors through the approved IAEA Training Programme:

    • Implementation of Phase II of the Interregional Training Course in Singapore and in Buenos Aires using the IAEA/NUS Curriculum
    • Training of Orthopaedic Surgeons and Transplant Co-ordinators through the MTF/IAEA Training Course and Transplant Co-ordinators through the Transplant Procurement Management (TPM) IAEA training programme.
    • First IAEA/NUS Internet Training Course for Tissue Banks Operators, Managers and Doctors in Singapore, Buenos Aires and Korea.

  3. To upgrade the quality of work in selected Tissue Banks in the participant countries with the aim of achieving international level.
  4. To support the activities of the Steering and the Technical Advisory Committees.
  5. To establish a Partnership Programme with other international and national organizations.

4. EXPECTED RESULTS

  1. Application of the International Standards, the Code of Practice and the Public Awareness Strategies in at least 70% of the participating countries by the end of 2004.
  2. More than 30 Tissue Bank Operators, Managers and Medical Doctors trained through the different training courses within the IAEA Training Course associated with the IAEA Radiation and Tissue Banking Programme at the end of 2004.
  3. More than 5 Tissue Banks achieving international level at the end of 2004.
  4. At least two meetings of the Technical Advisory and Steering Committees and the implementation of the recommendations approved by both Committees.

5. WORK PLAN FOR 2003-2004

Activity No. 1 Training Workshop for the Application of the International Standards for Tissue Banks.

Participants One candidate from each of the participant countries in the IAEA Radiation and Tissue Banking Programme.
Conditions to Participate The country presenting the candidate should express their willingness to introduce in their regulations the International Standards in all Tissue Banks participating in the Program after the conclusion of the Workshop.
Objective to be Achieved To train Tissue Bank staff in the application of the International Standards prepared by the IAEA.
Outcome of the Activity The Tissue Bank staff trained in the application of the International Standards in the participating Tissue Banks.
Contribution of the Host Country To provide secretariat support, room for the Workshop, transportation from the airport to the city and vice versa, local transportation and the use of PC, photocopying machines and other equipment needed in the Workshop.
Contribution of the IAEA: The air tickets and DSA and local expenses of the participants. If requested, the IAEA could provide limited amount of money to support the Workshop.

Activity No. 2 Training Workshop for the Application of the Code of Practice for the Radiation Sterilization of Tissue Allografts

Conditions to Participate The country presenting the candidate should express their willingness to promote the use of the Code of Practice in the facilities used by the Tissue Bank for the irradiation of tissues.
Objective to be Achieved To train Tissue Bank staff in the application of the Code of Practice by the irradiation facilities that irradiate the tissues.
Outcome of the Activity The Tissue Bank and the Irradiation Facility staffs trained in the use of the Code of Practice prepared by the IAEA.
Contribution of the Host Country To provide secretariat support, room for the Workshop, transportation from the airport to the city and vice versa, local transportation and the use of PC, photocopying machines and other equipment needed in the Workshop.
Contribution of the IAEA: The air tickets and DSA and local expenses of the participants. If requested, the IAEA could provide limited amount of money to support the Workshop.

Activity No. 3 Training Workshop for the Application of the Public Awareness Strategies for Tissue Banks

Contribution of the IAEA: The air tickets and local expenses of the participants. If requested, the IAEA could provide limited amount of money to support the Workshop.

REMARKS: Selection of the examiners. Two examiners will be provided by the IAEA.
  With this activity the training course initiated in April 2002 (Phase I) will be closed. The students that approved the course will receive the University Diploma.

Activity No. 5 Implementation of Phase II of the IAEA/NUS Interregional Training Course for Tissue Bank Operators, Managers and Medical Doctors.

Contribution of the IAEA: To facilitate the implementation of the course, including the selection of students and the payment of the fee to a limited number of them.

REMARKS: Selection of students will be made by the IAEA in consultation with the Director of the International (Regional) Training Center.
Financial support in the implementation of the course will be given by the IAEA to a limited number of students.
  Through this activity the Internet TC in Singapore, acting now as the International Training Center for the IAEA Radiation and Tissue Banking Programme, in Buenos Aires, acting as the Regional Training Center for Latin America, and in Republic of Korea, acting as a National Training Center, will be launched.

Conditions to Participate To be an Orthopaedic Surgeons or a Transplant Co-ordinator working or associated with a Tissue Bank, graduated from the Singapore or Buenos Aires Training Course. In the case of TPM there is no need to be graduated from previous courses.
Objective to be Achieved To train Orthopedic Surgeons and Transplant Co-ordinators working or associated with Tissue Banks following a specific training programme prepared jointly by MTF and the IAEA. In the case of TPM to train Transplant Coordinators using their own training program.
Outcome of the Activity Orthopedic Surgeons and Transplant Co-ordinators trained.
Contribution of the Host Country Facilities to implement the training programme and the payment of all local cost in the case of MTF. In the case of TPM their contributions need to be defined.
Contribution of the IAEA: MTF course: air tickets for the participants.

REMARKS: Selection of Candidates
Financial Support
  This activity is related with the implementation of the MOU between MTF and the IAEA as well as the Summary of the Agreements reached between these two organizations. The number of candidates to be selected will depend on the financial support to be provided by MTF and the resources available in the IAEA.
  A similar situation will exist with respect to the training of Transplant Co-ordinators in the TPM in Spain.

Follow up of the recommendations adopted by the Technical Advisory Committee and approved by the IAEA Steering Committee.
  It is expected that the Technical Advisory Committee will meet at least once a year. The Steering Committee will meet as required.
